Transitional Kindergarten/Kindergarten requirements
See below for requirements and please refer to the information you received during kindergarten round up.
- Up-to-date immunization card is required prior to first day of school
- School physical assessment completed by your provider before school begins
- Annual Student Health Update form - complete this form online yearly on JMC
- Vision screening form
- Dental Screening Certificate
- Lead Level - this is indicated on your school physical assessment
Elementary Health Information
Annual Health Update
- Parents must complete the Annual Student Health Update in JMC during registration each year before school begins.
Medication at School
- Medication Consent Form – If your child will take medication at school, please complete the Medication Consent Form and bring it to the office.
- Please report any new medical conditions or medication changes in the health update and contact the school nurse.
Vision Screening – 3rd Grade
- Vision Screening Form - All 3rd grade students are required to have a Vision Screening completed.

Allergies
- Allergy Policy
- Medication Consent Form – required if your child takes any medication at school.
- Food Allergy Diet Modification – complete this form if your child has a food allergy or cannot have certain foods at school.
- Allergy action plan – required yearly for any child with a severe allergy, completed by a physician.
- FARE : Food Allergy Anaphylaxis Emergency Care Plan
- Asthma Action Plan – recommended for any child with asthma.
- Seizure action plan – required yearly for any child with a seizure disorder.
- Self Carry Med Permission form – required if your child carries their own inhaler or EpiPen.
- Diabetes Medical Management plan
- Immunization Requirements – Please review the current Immunization Requirements for school attendance.
